Job Description:

The Certified Nursing Assistant (CNA) provides compassionate, person-centered care and support services to clients in home and community-based settings. The CNA assists clients with activities of daily living (ADLs), monitors client condition, promotes independence, and supports overall health and safety in accordance with Freedom Callers, LLC policies, care plans, and state regulations.

Essential Duties and Responsibilities:

Personal Care Services
• Assist clients with bathing, grooming, dressing, toileting, and personal hygiene
• Assist with mobility, transfers, positioning, and ambulation
• Assist with feeding and meal preparation
• Perform light housekeeping and laundry related to client care
• Provide medication reminders as permitted by agency policy
Health Monitoring & Safety
• Observe and report changes in client condition, behavior, or environment
• Measure and document vital signs when required
• Follow infection control and universal precaution procedures
• Maintain a safe and clean environment for clients
• Respond appropriately to emergencies and report incidents immediately
Documentation & Communication
• Complete documentation accurately and timely
• Maintain confidentiality and HIPAA compliance
• Communicate effectively with supervisors, nurses, clients, and family members
• Report accidents, incidents, concerns, or suspected abuse/neglect immediately
Professional Responsibilities
• Demonstrate professionalism, compassion, and ethical conduct
• Follow all Freedom Callers, LLC policies and procedures
• Attend required trainings, meetings, and in-services
• Maintain current CNA certification and competencies
