Position Summary
The Personal Care Assistant (PCA) provides compassionate, supportive, and person-centered care to clients in their
homes and community settings. The PCA assists clients with activities of daily living (ADLs), promotes independence,
ensures safety, and supports overall health and wellness in accordance with Freedom Callers, LLC policies and
individualized care plans.
Essential Duties and Responsibilities
Personal Care Assistance
• Assist clients with bathing, grooming, dressing, toileting, and hygiene
• Assist with mobility, transfers, and ambulation
• Provide meal preparation and feeding assistance when needed
• Assist with light housekeeping and laundry
• Support medication reminders as permitted by agency policy
Client Support & Safety
• Monitor client condition and report changes immediately
• Maintain a safe and clean environment
• Follow infection control and universal precautions
• Respond appropriately to emergencies and incidents
• Support community inclusion and engagement activities
Documentation & Communication
• Complete documentation accurately and timely
• Maintain confidentiality and HIPAA compliance
• Communicate effectively with supervisors, clients, and family members
• Report accidents, incidents, concerns, or suspected abuse immediately
Professional Standards
• Demonstrate professionalism, compassion, and ethical behavior
• Follow all agency policies and procedures
• Attend required trainings and in-services
• Maintain current certifications and competencies
