The Registered Nurse must:

  • Hold a current, unrestricted Registered Nurse license in the State of South Carolina.
  • Be a graduate of an accredited nursing program.
  • Have current CPR certification.
  • Have experience in home care, personal care services, community-based services, Medicaid services, geriatrics, behavioral health, or a related setting preferred.
  • Have strong assessment, documentation, communication, and organizational skills.
  • Be able to work independently in clients' homes and exercise appropriate clinical judgment.
  • Be comfortable supervising and providing guidance to direct-care staff.
  • Have reliable transportation and the ability to travel to client homes within the agency's service area.
  • Maintain all required credentials, screenings, training, and documentation required by Freedom Callers and applicable regulations.

Physical Requirements

The RN must be able to travel to clients' homes, enter and move safely within residential environments, sit, stand, walk, bend, and perform nursing assessments and other duties necessary for supervisory visits. Reasonable accommodations may be provided when applicable.

Work Schedule

This is a PRN/as-needed position. Hours are not guaranteed and will vary based on client census, required supervisory visits, reassessments, new admissions, and other clinical needs.

The RN must be reasonably available to complete assigned visits within required timeframes and submit completed documentation promptly following each visit.

Job Description:

Position Summary

Freedom Callers, LLC is seeking a dependable and professional Registered Nurse (RN) to provide PRN nursing oversight primarily through in-home supervisory visits. The RN is responsible for evaluating client care and service needs, monitoring the quality and appropriateness of services provided, supervising direct-care staff from a clinical perspective, and ensuring required nursing documentation is completed accurately and timely.

This is a PRN field-based position with visits scheduled according to client needs, regulatory requirements, and agency scheduling.

Essential Duties and Responsibilities

The PRN Registered Nurse will:

  • Conduct required initial, supervisory, reassessment, and follow-up visits in clients' homes as assigned.
  • Assess the client's current condition, functional abilities, care needs, safety concerns, and overall well-being.
  • Review the client's plan of care and determine whether services continue to meet the client's needs.
  • Identify and document changes in the client's condition and communicate significant concerns to the Director/Administrator and other appropriate parties.
  • Observe caregiver performance during supervisory visits when applicable and evaluate whether services are being provided safely and according to the client's plan of care.
  • Provide instruction, coaching, and education to caregivers when deficiencies or additional training needs are identified.
  • Review client records and documentation for completeness, accuracy, and compliance with agency requirements.
  • Complete all required nursing assessments, supervisory visit notes, reassessments, and other clinical documentation within required timeframes.
  • Assist with updating plans of care when a client's condition, needs, or services change.
  • Report suspected abuse, neglect, exploitation, unsafe conditions, significant changes in condition, and other reportable concerns according to agency policy and applicable requirements.
  • Participate in incident review and follow-up when nursing assessment or clinical oversight is required.
  • Communicate professionally with clients, families, caregivers, case managers, physicians, and other members of the client's care team as appropriate.
  • Provide nursing consultation to agency management regarding client care and caregiver concerns.
  • Assist with staff competency assessments and clinical training when requested and when within the RN's scope of practice.
  • Provide medication-related oversight, education, or documentation review when applicable to the client's service and within agency policy and the RN's scope of practice.
  • Maintain client confidentiality and comply with HIPAA requirements.
  • Follow all Freedom Callers policies and procedures and applicable federal, state, Medicaid, payer, and licensing requirements.
  • Perform other nursing-related duties consistent with the RN's scope of practice as assigned.
